Important Community Health Survey announcement
for Currituck County residents!

Chesapeake Regional Healthcare is conducting a community health needs assessment to collect vital information about the health and quality-of-life needs of those who live and work in the areas that the hospital serves.

The online survey should only take you about 12 minutes to complete.

Your answers will remain anonymous and will provide important information to the hospital regarding the present and future of its community health services.

Physical copies of the survey will be available at locations around Currituck County throughout October.

Once you complete the survey, you'll have the option to submit contact information and be entered into a drawing to win a $50 gift card!
